This form is needed for every patient (new or previous) that wants to get a contact lens prescrption in addition to a comprehensive exam and glasses prescription.
If you would like us to have your records from your previous doctor you can print out this form and fax it to them prior to your appointment. Please follow-up with us to make sure we have received them before you arrive.
Your privacy is important to us and this form explains the only ways in which your information will be shared without your prior consent. This form is also available in our office upon request
